Participation & Assumption of Risk Agreement

Because this service includes yoga and movement, each individual will need to complete the Liability Waiver before participating. This form simply ensures everyone understands the inherent risks of yoga and takes personal responsibility for their health and readiness. Please review and submit it prior to beginning the program, session, workshop, or class.
